    Maternal mortality in a tertiary care hospital: a 3-year retrospective study

    Background: Systematic review to determine the epidemiological aspects and causes of maternal mortality there by exploring possibility of intervention and implementing Evidence-based health policies and programmes to prevent future maternal death. Aims and objectives of the study were to calculate the maternal mortality rate in our hospital, to assess the epidemiological aspects of maternal mortality, to assess the type of delay and causes of maternal mortality and to suggest ways to reduce the MMR.Methods: This is a 3-year retrospective study from January 2017 to December 2019 that will be conducted in the department of obstetrics and gynaecology, The Apollo medical college and District hospital, Chittoor a tertiary care teaching hospital situated in the southernmost part of Andhra Pradesh state in India. It gets a large number of referrals from PHCs, CHCs, and maternity homes as well as from hospitals across Chittoor district. Epidemiological data will be collected from the hospital register. Maternal mortality ratio, epidemiological factors and causes affecting maternal mortality are assessed.Results: MMR in present study was 66 per 1,00,000 live births. Women in the age group of 20 to 30 years (85.72%), illiteracy (57.16%) and low socioeconomic status (100%) were risk factors for maternal mortality. Obstetric haemorrhage (57.16%) is most common cause whereas type 1 and type 2 delays are most common contributing factors for maternal mortality.Conclusions: Early identification and management of pregnancy complication, strengthening of existing Emergency obstetric care (EmOC) facilities, easy transport and appropriate referral linkages are keys to reduce maternal mortality to further extent

    Incidence and risk factors associated with ectopic pregnancy: a prospective study

    Background: Increased incidence of ectopic pregnancy and its impact on women’s fertility in recent years need significant attention.Methods: A two years prospective study from January 2018 to December 2019 conducted to determine incidence, association of risk factors with ectopic pregnancy and find the most common risk factor of ectopic pregnancy in department of obstetrics and Gynaecology, a tertiary care Hospital in Pune.Results: During the study period 100 patients were diagnosed to have ectopic pregnancy. Incidence was 5.29 per 1000 births. Majority were in the age group of 20-24 years (42%), multiparous (59%) and belong to low socioeconomic state (62%). In majority of the patients (22%) no risk factors was found. Among the patients who had risk factors, the main risk factors for ectopic pregnancy were history of history of pelvic inflammatory disease (20%), previous tubal/abdominal surgery (12%), history of Infertility (10%), previous termination of pregnancy (10%), contraception with mirena IUS or IUCD in situ (8%) and a history of prior ectopic pregnancy (4%).Conclusions: In majority (78%) of patients risk factors for ectopic pregnancy was present and pelvic inflammatory disease was found to be a major risk factor for ectopic pregnancy

    An Inverse Scattering Transform for the Lattice Potential KdV Equation

    The lattice potential Korteweg-de Vries equation (LKdV) is a partial difference equation in two independent variables, which possesses many properties that are analogous to those of the celebrated Korteweg-de Vries equation. These include discrete soliton solutions, Backlund transformations and an associated linear problem, called a Lax pair, for which it provides the compatibility condition. In this paper, we solve the initial value problem for the LKdV equation through a discrete implementation of the inverse scattering transform method applied to the Lax pair. The initial value used for the LKdV equation is assumed to be real and decaying to zero as the absolute value of the discrete spatial variable approaches large values. An interesting feature of our approach is the solution of a discrete Gel'fand-Levitan equation. Moreover, we provide a complete characterization of reflectionless potentials and show that this leads to the Cauchy matrix form of N-soliton solutions

    Iris Recognition Approach for Preserving Privacy in Cloud Computing

    Biometric identification systems involve securing biometric traits by encrypting them using an encryption algorithm and storing them in the cloud. In recent decades, iris recognition schemes have been considered one of the most effective biometric models for identifying humans based on iris texture, due to their relevance and distinctiveness. The proposed system focuses on encrypting biometric traits. The user’s iris feature vector is encrypted and stored in the cloud. During the matching process, the user’s iris feature vector is compared with the one stored in the cloud. If it meets the threshold conditions, the user is authenticated. Iris identification in cloud computing involves several steps. First, the iris image is pre-processed to remove noise using the Hough transform. Then, the pixel values are normalized, Gabor filters are applied to extract iris features. The features are then encrypted using the AES 128-bit algorithm. Finally, the features of the test image are matched with the stored features on the cloud to verify authenticity. The process ensures the privacy and security of the iris data in cloud storage by utilizing encryption and efficient image processing techniques. The matching is performed by setting an appropriate threshold for comparison. Overall, the approach offers a significant level of safety, effectiveness, and accuracy

    The Li0.5La0.5TiO3 and LiFePO4/C has been prepared using sol-gel method for its interface analysis towards its application for all solid state assembly. The LiFePO4cathode material and Li0.5La0.5TiO3 solid electrolyte has been individually tested for its electrochemical reversibility. The thin film battery assembly shows severe capacity fading, which results in electrochemically non active with the chosen anode materials. In order to verify the electrochemical activity and interaction of Li0.5La0.5TiO3-LiFePO4/C interface the mixture has been tested for its electrochemical reversibility. The Li0.5La0.5TiO3-LiFePO4/C interface exhibits well resolved oxidation-reduction hype which verifies its suitability towards all solid state assemblies

    CLINICAL EVALUATION OF A SIDDHA POLY HERBAL FORMULATION AZHINJIYATHIKASHAYAM IN MADHUMEGAM (TYPE - 2 DIABETES) PATIENTS

    Azhinjiathikashayam is a herbo mineral Siddha poly herbal formulation said for the management of Madhumegam (Type-2 Diabetes mellitus) in ancient Siddha literature. The study drug was subjected to estimate the therapeutic efficacy in Madhumegam patients. A well designed study protocol was approve by Institutional Ethics Committee and the clinical study was conducted as an open clinical trial in OPD/IPD of Ayothidoss pandithar Hospital of National Institute of Siddha Chennai. Based on the inclusion criteria 40 Madhumegam patients (Type -2 Diabetes mellitus) were enrolled in this study. Informed consent was obtained from each patient before study initiation. 60ml of Azhinjiathikashayam was administered orally before food twice a day for a period of 90 days and advised to follow the prescribed dietary regimen. All the baseline data, Laboratory investigations were recorded in the prescribed Case Report Form of each patient. Blood sugar level in fasting and postprandial was done once in 30 days and HbA1c was done before treatment (0th day) and after treatment (90th day). The clinical assessment was recorded once in 10 days. Paired t test was used to test the significance of treatment using before and after treatment data on HbA1c, Clinical symptoms, Blood sugar fasting and postprandial. The level of significance probability 0.05 was used to test the treatment difference and the values are statistically significant. In this study there is statistically significant difference between before and after treatment on average HbA1c, average blood sugar of fasting and average clinical feature score (p < 0.0001)

    Prescription audit of outpatients in tertiary care government hospital

    Background: Prescription audit is a tool as well as a technique by its application,all professionals will improve the quality of prescribing drugs. Standards of medical treatment can be assessed by prescription audit. It is based on documented evidences to support diagnosis, treatment and justified utilization of hospital facilities. Prescription audit is a quality improvement process that seeks to improve patient care. In this background the present study was conducted. The objectives of the study were to know the frequently prescribed drugs in OPD, number of the drugs used per prescription and to find out the rationality.Methods: Study was conducted at Sri Chamarajendra Hospital, HIMS, Hassan in OPD of General Medicine.1000 prescriptions were collected and noted down the frequently used medication, number of drugs prescribed and their type of formulations for the particular diagnosis.Results: From the study it is noted that 1910 drugs out of 1000 prescriptions were prescribed which is approximately 1.91 drugs per prescription about 55% of the prescriptions contained single drug. Very few received 4-5 drugs (7%). Almost all the drugs in prescriptions were in Generic names. Around 95% of prescriptions doses were mentioned in mg, ml etc. The most commonly prescribed drugs in order are Antibiotics, antidiabetics antihypertensives, bronchodilators, steroids antiemetics and ORS were prescribed.Conclusions: Polypharmacy was not found in our prescriptions which indicates our prescriptions improved the patient conditions. This type of study will ensure to know the ‘P’ drug development and select the essential medicine list for various levels of health care

    Treatment adherence and factors contributing to non adherence among type 2 diabetes mellitus patients in a tertiary care hospital: a cross sectional study

    Background: Diabetes mellitus (DM) is the most common endocrine disorder and major global public health problem. Lack of adherence to antidiabetic medication has lead to suboptimal blood sugar control, treatment failure, accelerated development of complications and increased mortality, thus medication adherence plays an important role in disease control. Hence present study was taken to evaluate the treatment adherence and factors affecting non adherence among Type 2 diabetes mellitus patients.Methods: A cross sectional study carried out by Department of Pharmacology and Medicine, Hassan Institute of Medical Sciences, Hassan. Total 150 patients of type 2 diabetes were recruited after taking their informed consent. Adherence to treatment and factors associated with non adherence has been assessed during a personal interview with each patient using standardized questionnaire.Results: Adherence levels were 28%, 42% and 30% for high, medium and poor adherence respectively. The overall prevalence of non adherence among respondents was 30%. Among them 77.77% were males, 44.44% belonged to age group of 41-60 years, 40% illiterate, 60% employed, 51.11% of patients with smoking and alcoholic habits were not adherent to anti diabetic treatment. Other reasons contributing to non-adherence to treatment were forget fullness (86.66 %), inadequate knowledge about side effects (80%), unhappy clinical visits (71.11 %) and lack of assistance (48.88 %).Conclusions: Results showed that patients in the area of study were moderately adherent to anti-diabetic medications. This emphasizes the need for constant motivation and education at frequent intervals to ensure better adherence

    Squeezing of a coupled state of two spinors

    The notion of spin squeezing involves reduction in the uncertainty of a component of the spin vector below a certain limit. This aspect has been studied earlier for pure and mixed states of definite spin. In this paper, this study has been extended to coupled spin states which do not possess sharp spin value. A general squeezing criterion has been obtained by requiring that a direct product state for two spinors is not squeezed.
